Abstract

Moyamoya disease is a cerebrovascular disease of unknown etiology, which is characterized by bilateral stenosis or occlusion at terminal portion of internal carotid artery and at proximal portion of anterior cerebral artery and /or middle cerebral artery and abnormal vascular network in the vicinity of the arterial occlusions. It occurs frequently in Asian countries, particularly in Korea and Japan, but is rare in Western countries. To establish the etiology of moyamoya disease, much about the pathology from autopsies, factors involved in its pathogenesis, and its genetics have been studied. It may occur at any age from childhood to adulthood and in general, initial manifestation is cerebral ischemic symptoms in children and intracranial hemorrhage symptoms in adults. Because it progress and cause recurrent stroke, early diagnosis and proper management has been recognized. Cerebral angiography is essential for definitive diagnosis and treatment plan. Magnetic resonance imaging/magnetic resonance angiography is useful for diagnosis and follow-up tools after revascularization. Evaluation of the cerebral hemodynamics by single photon emission computed tomography and positron emission tomography is useful for diagnosis and assessment of the severity of cerebral ischemia in moyamoya patients. Surgical revascularization is effective for moyamoya disease manifesting as ischemic symptoms, to prevent further ischemia and infarction. In hemorrhagic type moyamoya disease, revascularization can be considered. Direct bypass, indirect synangiosis and combined methods are used. Outcomes of revascularization are excellent in preventing transient ischemic attacks in most patients.

서 론

모야모야병(moyamoya disease)은 특별한 원인 없이 내경동맥의 원위부나 그 분지인 중대뇌동맥이나 전대뇌동맥의 근위부에 협착이 생기고 점차 진행하여 폐색이 일어나고 모야모야 혈관이라고 하는 비정상적인 이상혈관이 뇌기저부에 관찰되는 만성 진행성 뇌혈관 질환이다. 서양보다는 동양 특히 동아시아에서 많이 발생하는 것으로 알려져 있다. 1957년 일본에서 처음으로 증례가 보고되었으며 1960년대 병 이름으로 “모야모야”라는 용어가 사용되었으며 체계적으로 뇌혈관조영술의 소견과 임상증상에 대해 기술되기 시작하였다[1]. ‘모야모야’는 일본말로 뇌혈관조영술에서 뇌기저부의 비정상적인 이상혈관이 마치 모락모락 피어오르는 담배연기(hazy cloud like puff of cigarette smoke) 처럼 보인다고 해서 붙여진 이름이다. 뇌동맥의 협착과 폐색이 양측으로 진행하면 모야모야병으로 확진하며, 일측성으로 한쪽에만 생기면 모야모야병으로 추정한다. 뇌혈관폐색과 관련하여 비슷한 용어가 있는데, 뇌동맥 죽상경화증, 혈관염, 자가면역질환 등 혈관 폐색을 일으킨 원인이 있을 때는 모야모야증후군이라고 한다[2,3]. 어린이와 어른에게서 모두 발병하는 것으로 알려져 있으며 어린이는 주로 반복적인 일과성 뇌허혈 발작(transient ischemic attack)과 뇌경색의 형태로 나타나고 어른에게서는 뇌출혈이 흔하다[4,5].

모야모야병은 아직 그 원인이 알려져 있지 않아 이 질환을 막을 수 있는 근본적인 치료법은 없다. 뇌동맥 폐색에 대한 수술적 치료인 혈관 문합술의 효과는 여러 보고를 통해 입증되어 있다[6,7]. 이에 본 연구에서는 지금까지의 모야모야병에 대한 연구들을 검토해보고 일반적으로 받아들여지는 역학, 병리소견, 임상증상, 진단방법, 치료 및 예후에 대해 알아보고자 한다.

본 론

1. 역학병리

모야모야병 발병의 지역적 차이는 매우 커서 한국, 일본 등의 극동아시아에서 호발하고 유럽과 미대륙의 사람들에는 드물다. 한 연구에서는 세계에서 환자의 분포를 조사하였는데 일본을 제외하고 1,063명의 환자 중 아시아 625명(한국 289명, 중국 245명), 유럽 201명, 남북 아메리카 176명으로 아시아에서 흔하게 발병하는 것으로 알려졌다[8].

일본에서의 대규모 역학연구에 따르면 인구 10만 명당 3.16명이 이 병을 가지고 있으며 0.35명이 발병한다[9,10]. 남녀의 비율은 1:1.8∼1.9로 여자에게서 많이 발병한다[9]. 환자의 10%에서 가족력이 있으며[9,10], 일반인에 비해 환자의 형제는 30배 정도 환자의 자녀는 40배 정도 발병의 위험성이 높다[9]. 증상이 발생하는 연령은 10세 이전에 가장 잘 생기고 30대에 두번째로 잘 발병하는 것으로 알려져 있다[9-11]. 모야모야병으로 인한 출혈은 50대에 가장 잘 생기는 것으로 알려져 있다[12]. 최근 건강검진 등으로 뇌 자기공명영상(magnetic resonance imaging, MRI)을 많이 시행함으로 인해 증상이 없는 모야모야병이 진단되고 그 비율이 점차 증가하고 있으며 최근 역학조사에서는 증상 없는 모야모야병의 유병률이 인구 10만 명당 10.5명으로 조사되었다[13].

부검을 통하여 모야모야병의 주된 소견이 내경동맥의 말단부의 협착과 폐색, 모야모야 혈관의 형성, 측부순환의 형성이다. 조직학적 소견으로 내경동맥 내막의 증식, 비후된 내막의 지방축적, 내탄력층의 물결모양의 변형, 윌리스환 주위의 작은 혈관 채널, 연막에 작은 혈관채널이 특징적이다[14].

모야모야병의 원인과 관련하여 여러 인자에 대해서 연구가 진행되고 있는데 basic fibroblast growth factor, transforming growth factor-1, platelet derived growth factor, interleukin-1, -2, cellular retinoic acid- binding protein-I 등의 인자와 수용체가 모야모야병 환자의 표재측두동맥(superficial temporal artery), 혈액 혹은 뇌척수액에서 과발현됨이 보고되었다[15,16]. 가족성 모야모야병에서 유전자의 위치가 3p24-p26, 8q25, 6q25, 17q25 등 여러 곳으로 알려졌으며, 3세대 이상에 걸쳐 상염색체 우성으로 유전된다[17]. 일측성의 모야모야병이 양측으로 진행하는 환자의 유전자 검사를 통해 가족성 모야모야병과 비교하여 이 병을 진행하게 하는 유전자의 위치를 찾는 노력이 진행되고 있다.

2. 임상증상 및 징후

처음 증상에 따라 출혈형, 간질형, 뇌경색형, 일과성 허혈형, 빈번한 일과성 허혈형, 두통형, 무증상형, 기타로 분류하는데 일과성 허혈형이 가장 많고 출혈형, 뇌경색형, 빈번한 일과성 허혈형, 두통형의 순서로 많다[10].

증상은 연령과 병의 진행 형태에 따라 아주 다양하게 나타난다. 뇌허혈에 의한 증상은 어린이에서 많이 나타나는데 어린이 환자의 약 70%를 차지한다. 특히 3세 이하의 어린이에서는 병의 진행이 빠르며 뇌경색이 많이 발생하는데 뇌출혈은 거의 생기지 않는다[10]. 이러한 뇌허혈성 증상은 과호흡 후에 많이 유발되는데 심한 운동을 하거나 심하게 울 때, 하모니카를 불거나 뜨거운 음식을 불며 식히는 과정에서 자주 발생하게 된다. 뇌허혈 증상으로는 운동마비, 의식저하, 감각이상, 간질, 두통, 언어장애, 감각 이상 등이 흔하고 이런 증상은 갑자기 생기며 주로 같은 쪽으로 반복되고 진행하기도 한다. 병이 진행하여 뇌허혈증상이 반복되어 뇌위축이 유발되고 뇌경색으로 악화되어 심각한 지능장애를 초래한다.

어른에게서는 주로 뇌출혈이 생기는데 여자에게서 출혈이 잘 생기는 것으로 알려져 있으며 의식저하, 두통, 운동마비, 언어장애가 나타나며 뇌실내출혈이 많고 사망률이 높으므로 조심해야 한다.

두통은 어린이에게서 많이 나타나고 편두통처럼 심한 경우에서부터 묵직한 두통까지 다양하게 나타나며 아직 그 발생기전은 정확히 알려져 있지 않다[18].

3.진단

모야모야병의 진단에 뇌혈관조영술은 필수적이며 치료방침을 결정하고 수술결과를 평가하는 표준방법이다. 내경동맥 말단부나 전대뇌동맥이나 중대뇌동맥의 기시부의 폐색이 다양하게 나타나고 기저부에 비정상적인 작은 혈관들(모야모야혈관)이 보인다(Fig. 1). 모야모야병의 진행여부를 알아보는데도 뇌혈관조영술이 가장 유용하다(Fig. 2). 최근에는 MRI/magnetic resonance angiography (MRA)의 정확도가 높아짐에 따라 모야모야병의 진단에 이용되고 있다. 정확한 영상을 얻기 위해 1.5 T 이상의 기기로 검사할 것을 권장하고 내경동맥의 말단부나 전대뇌동맥이나 중대뇌동맥의 기시부의 협착이나 폐색이 보이며 기저핵주위로 비정상적인 혈관 채널 즉 모야모야혈관이 보이며 이런 소견이 양측으로 보일 때 확정적 모야모야병이라고 진단한다. 기저핵 주위에 2개 이상의 signal void가 보이면 비정상적 혈관채널이라고 판단할 수 있다(Fig. 3) [19].

모야모야병의 뇌허혈 정도를 측정하고 치료방침과 예후를 결정하기 위해서는 양전자방출단층촬영(positron emission tomography, PET)이나 단광자방출단층촬영(single photon emission computed tomography, SPECT)을 이용한 뇌혈류 및 뇌대사 상태를 측정해야 한다.

뇌허혈상태에서 뇌산소대사를 유지하기 위한 보상작용이 일어나는데, 모야모야병에서 뇌동맥의 폐색으로 인한 뇌관류압(cerebral perfusion pressure)이 심하게 감소되어 뇌혈관의 확장반응만으로 뇌혈류(cerebral blood flow, CBF)를 유지할 수 없게 되어 보상적으로 뇌혈류량을 증가시키게 되고 산소추출율은 증가시킨다[20,21]. PET에서는 국소 뇌혈류(regional cerebral blood flow)와 국소 산소 대사율(regional cerebral metabolic rate of oxygen)이 감소하고 반면에 국소 뇌혈류량(regional cerebral blood volume)과 국소 산소추출률(regional oxygen extraction fraction)은 증가된다[20,21]. SPECT는 국소 뇌혈류를 알 수 있고, acetazolamide 정맥 주사 전후를 비교하면 혈류 예비능(vascular reserve)을 측정할 수 있다(Fig. 4) [22]. 수술 전후의 결과를 비교하여 수술의 효과를 알아보는데도 유용하게 사용할 수 있다. 경두개도플러(transcranial Doppler), perfusion MRI, perfusion CT도 뇌혈류상태를 평가하는데 이용되고 있다.

모야모야병의 볼 수 있는 특징적인 뇌파는 과호흡시에 규칙적인 고진폭의 서파가 출현하고(build-up), 과호흡이 끝난 수분 후에 불규칙해진 고진폭의 서파가 다시 나타나는(rebuild-up현상) 것으로 어린이에게서 잘 나타난다. 이 rebuild-up 현상은 과호흡에 의해 유발된 과이산화탄소혈증에 의해서 대뇌피질의 뇌혈류가 회복이 늦어져 생기는 것으로 추측된다[23].

4.치료

Chiu 등[24]의 연구에 의하면 치료하지 않은 모야모야병의 장기추적결과 매년 10.3%의 뇌졸중 발생률을 보였는데 뇌졸중은 진단 첫해에 18%, 그리고 그 이후는 매년 5%의 발생률을 보였다. 뇌혈관의 폐색정도도 진행하고 그 진행정도에 따라 출혈이나 뇌경색이 더 잘 생기며 재출혈의 위험도 높아진다[5,25,26].

모야모야병이 진행함에 따라 심각한 신경학적 이상 증상을 초래할 수 있어서 치료를 고려하는데 치료의 목표는 반복되는 일과성 뇌허혈 발작을 막고, 병이 진행되지 않도록 하여 뇌경색을 예방하고 뇌출혈을 막는 것이다. 뇌혈류량과 혈류 예비능을 호전시키기 위해서는 보존적인 치료보다 재관류술인 수술적 치료가 더 효과적인 것으로 알려져 있다[20,27].

모야모야병은 환자 상태에 따라 수술 시기와 수술 방법 및 그 적응증이 달라진다. 뇌허혈 발작환자에서 광범위한 뇌경색이 없고 회복이 가능한 뇌허혈에 의한 증상만 있으며 SPECT, PET 등의 뇌혈류 검사에서 뇌혈류량이 감소되어 있고 acetazolamide 정맥주사 검사로 혈류 예비능이 저하되어 있을 때 수술로 좋은 효과를 볼 수 있다.

증상이 없는 모야모야병이라도 진행의 가능성이 높은 어린이에게서 뇌혈류검사 결과에 따라 수술하는 경우가 있으나 어른에게서는 권유되지 않는다[28]. 한쪽 뇌혈관이 막힌 추정 모야모야병의 경우, 2세 이하의 어린이에서는 양측성으로 진행하는 경우가 많으나 나머지 환자군에서는 이러한 진행을 보이지 않는 경우도 있어 증상이 있는 반구에 대해서만 수술을 해주고 나머지 반구에 대해서는 집중적 추적관찰을 하는 것이 권유된다[29]. 3세 이하의 어린이의 경우 병의 진행이 더 빨라 발견 당시 뇌경색이 있는 경우가 많고 수술을 기다리는 기간 동안에도 추가적인 뇌경색이 발생하는 경우가 39%까지 나타나 조기에 수술을 해주는 것이 필요하다[30,31].

성인 모야모야병에서 출혈성 뇌졸중의 발생은 지속적인 혈역학적 과부하에 노출된 기저 모야모야혈관의 파열에 의한 것으로 알려져 있다. 그래서 주로 기저핵, 시상 혹은 뇌실 주변의 백질에서 출혈이 시작되고, 뇌실내 출혈로 확장되는 경우가 빈번하다[32,33]. 혈관촬영에서 측부순환의 혈관이나 모야모야 혈관에서 미세 동맥류가 발견되는 경우도 있다. 모야모야병의 측부순환에 중요한 역할을 하는 후순환계 뇌동맥에서 지속적인 혈역학적 스트레스에 의한 낭성 뇌동맥류가 잘 발생하고 이로 인한 뇌지주막하 출혈의 발생도 드물지 않다[34].

출혈성 모야모야병은 특히 예후가 좋지 않아 재출혈률이 연간 7.09%로 보고되고 있다[26]. 장기적으로 재출혈을 예방할 수 있는 치료가 절실히 필요하고 직간접 혈관 재건술을 통한 측부순환의 발달이 궁극적으로 혈류 역학적 과부하를 경감시켜 재출혈을 예방할 수 있을 것이라는 기대 때문에 여러 기관에서 혈관 재건술을 시도하였지만 그 효과에 대해서는 대단히 의견이 분분하다. 다양한 종류의 혈관재건술을 시행하였지만 재출혈 예방에 유의한 실효를 거두지 못했다는 보고가 있는가 하면[7,35], 직간접 동시 재건술을 시행한 군에서 뚜렷하게 재출혈이 감소되는 경향이 있고 혈관촬영소견에서 말초혈관의 미세동맥류가 사라지거나 혈류 역학적 스트레스 안정화되는 현상을 관찰하였으므로 잠재적으로 유효할 가능성을 시사한 연구들이 발표되고 있기도 하다[25,36]. 출혈성 모야모야병 환자의 효과적인 치료법에 대한 대규모 연구결과는 아직 없으나, 2001년 일본 전국 23개 기관이 참여한 다기관 무작위 공동연구(Japanese adult moyamoya trial)가 시작되어 곧 발표될 결과에 관심이 크다.

직접 재관류(direct revascularization) 수술은 표재측두동맥과 중대뇌동맥의 피질 분지 사이의 문합을 통해 이루어지는데, 혈액관류가 적은 뇌 부분에 직접 재관류를 시켜줄 수 있고 수술 직후부터 혈류량이 증가된다는 장점이 있다. 반면에 혈관이 작으면 수술이 어렵고 국소 뇌혈류를 막아서 뇌경색이 유발되는 경우가 있으며 재관류가 이미 생성된 측부순환을 방해할 가능성도 있어서 주의해야 한다. 성인의 경우 신생혈관형성이 잘 되지 않아 간접 재관류 수술의 효과가 제한적이어서 직접 재관류술이 효과가 좋은 것으로 알려져 있다[37].

간접 재관류(indirect revascularization) 수술은 직접 재관류술에 비해 덜 침습적이며 수술 술기가 쉬운 장점이 있으나 신생혈관생성에 시간이 걸리며 목표로 한 대뇌피질에 재관류가 충분하지 못할 수 있는 단점이 있다. 간접 재관류술에는 여러가지 방법이 있는데 뇌경막, 측두근육, 중경막동맥(middle meningeal artery), 모상건막(galea aponeurotica), 표재 측두동맥과 같은 혈관 분포가 풍부한 표재 조직을 뇌 표면에 접촉하게 하여 신생혈관이 형성되도록 하여, 두개강 바깥의 외경동맥 분지와 두개강 안쪽의 내경동맥의 뇌피질 분지와 문합을 이루어 혈류가 통하게 하는 것이다[38]. 나이 어린 어린이의 경우 혈관이 너무 작아 직접 재관류술은 술기상 어려움이 많아서 간접 재관류술을 많이 사용하고 있다. 성인에서는 직접 재관류술을 많이 시행하는데 서로의 단점을 보완하기 위해 혼합형 재관류술을 시행하기도 한다. 중대뇌동맥의 영역에 주로 재관류술을 시행하였는데 인지기능에 중요한 역할을 하는 전대뇌동맥의 영역에도 혈류량을 증가시키기 위해 전두엽에 모상건막을 삽입하는 등 다양한 수술방법이 행해지고 있다.

모야모야병이 진행하여 후순환계(posterior circulation)까지 파급되면 후대뇌동맥영역에 뇌혈류를 증가시키기 위해 후두동맥을 이용한 encephalodural arterial synangiosis나 multiple burr holes 수술 등이 시행된다[39].

수술과 관련된 합병증으로는 뇌졸중, 뇌경막하수종 및 혈종, 과관류증후군, 창상감염 등이 있으며, 가장 문제가 되는 것은 수술 중 혹은 수술 후에 발생되는 뇌졸중이다[31,40]. 수술 후 뇌졸중은 3.8%에서 22.2%까지 보고되고 있다[31,40]. 이와 같은 합병증을 예방하기 위하여는 수술 중에 정상 이산화탄소 분압, 정상혈압, 정상체온을 반드시 유지하고 충분한 수액공급과 빈혈교정의 필요성을 강조하였다. 그 외에도 수술직후 수술통증으로 심하게 울면서 초래되는 과호흡상태 때문에도 뇌허혈증상이 나타날 수 있으므로 수술직후 통증관리에 세심한 배려가 필요하다.

5.예후, 추적검사

영아에서 뇌경색의 유무가 예후에 가장 중요한 요인이다. 일측성인 추정 모야모야병이 양측성으로 진행하고 증상이 없던 환자도 시간이 지남에 따라 약 65%에서 일과성 뇌허혈 발작이 발생한다[41]. 많은 연구에서 재관류술후 두통도 호전되며 일과성 허혈발작은 많이 줄어들거나 없어지고 뇌경색도 아주 감소하였으며 뇌기능도 호전되어 지능도 좋아지는 것으로 나타났다[18,24,34].

모야모야병에서 뇌출혈로 인한 사망률은 6.8%에서 20%까지 보고되고 있으며 30∼60%가 재출혈한다고 알려져 있다. 증상이 없는 모야모야병도 점차 진행하여 뇌경색이 40%에서 발생한다고 하며, 연간 출혈의 위험성은 3.2%로 보고되고 있다[42]. 따라서 증상이 없는 모야모야병도 뇌졸중의 위험이 있는 환자로 관리해야 할 것이다. 모야모야병의 추적조사시기와 방법에 대해서는 확실히 정립이 되지 않았다. 직접 재관류술을 시행한 경우 수술 후 6개월에 뇌혈관조영술과 SPECT를 시행하여 수술에 따른 효과를 확인해보는 것이 권장되고(Fig. 5), 간접 재관류술을 시행한 환자는 수술 후 6개월 2년에 뇌혈관조영술과 SPECT를 시행하는 것이 좋다[43,44]. 증상이 있으나 치료를 하지 않은 환자는 매년 MRI/MRA로 병이 진행하는지 알아보며 증상이 없는 모야모야병 환자는 주기적인 검사보다는 필요하면 그때 그때 MRI/MRA를 시행하는 것이 좋다. 새로운 증상이 생기면 반드시 뇌혈관조영술과 PET, SPECT를 시행하여 치료방침을 정해야 한다[44].

결 론

모야모야병의 발병기전이 아직 정확하게 규명되어 있지 않아서 원인 규명을 통한 예방, 조기진단, 진행방지 및 근원적 치료는 불가능한 실정이고 질환 자체가 비교적 최근에 알려져서 대부분의 연구가 단기 추적검사결과만 포함하고 있고 질병의 진행이 만성적인 경과를 취하고 다양한 임상경과를 보이기 때문에 장기적인 임상경과나 예후는 알려져 있지 않다. 더 많은 연구를 통해 모야모야병의 병인이 밝혀지고 치료법이 개발 될 것을 기대한다.
