Abstract

The incidence of renal cell neoplasms has been increased in worldwide as well as in Korea. Even though the World Health Organization (WHO) Classification of renal tumors (2004) is currently used, new entities require to be added in the updated classification because of recent modification with our understanding of the molecular biology and different clinical behavior of new renal tumors. In this review, recently described tumors and candidate entities will be discussed. It is of importance to know these new entities for the proper diagnosis, treatment, and their prognosis.

서 론

콩팥세포암종(renal cell carcinoma, RCC)은 다른 비뇨기암에 비해서는 드물지만 특히 남성암에서는 비교적 흔히 관찰되는 암이다. 2014년 미국 암 통계를 보면 남자에서는 6번째로 자주 발생하는 암으로 39,140명이 발병할 것으로 예상하고 있으며, 여성에서는 24,780명이 발병하여 8번째로 자주 발생하는 암에 해당된다고 보고하고 있다 (Tabel 1) [1]. 2011년 우리나라 통계(국가암정보센터)에서도 2,722명이 발병하여 남성 전체 암의 9번째를 차지하고 있다[2].

조직학적 5대 RCC는 clear cell (conventional) RCC, papillary RCC, chromophobe RCC, collecting duct RCC (CDC), unclassified RCC이다. Clear cell RCC 66~75%, papillary RCC 10~15%, chromophobe RCC 5%, CDC <1%, unclassdified RCC 5% 정도의 빈도를 보이고 있다. 그런데, 면역조직화학검사, 분자검사, 세포유전학적 분석방법의 급격한 발전으로 새로운 지식이 쏟아져 나옴에 따라 그 전에는 분류 할 수 없었던 RCC가 새로운 특징들을 보여 기존의 2004년 WHO 분류(Table 2) [3]에 해당되지 않는 종류의 RCC들이 보고 되기 시작하였다(Table 3). 이 종설에서는 최근 International Society of Urologic Pathology (ISUP)의 모임에서 논의되고 새롭게 기술된 RCC들(tubulocystic renal cell carcinoma, thyroid-like follicular renal cell carcinoma, acquired cystic disease associated renal cell carcinoma, clear cell [tubule-] papillary renal cell carcinoma)과 완전한 결론에 이르지는 못했지만 장래에 독립된 신장암의 아형이 될 가능성이 있는 잠정적인 RCC들(renal cell carcinoma with t(6;11), Hybrid oncocytoma/chromophobe renal cell carcinoma, hereditary leiomyomatosis and renal cell carcinoma syndrome, renal angiomyoadenomatous tumor)에 대해 기술한다. 병리 의사 뿐만 아니라 비뇨기과, 영상의학과, 종양내과, 치료방사선과 의사들이 진단과 치료, 예후 판정을 위해 이러한 새로운 질환을 아는 것은 매우 중요하다. 이 종설에서 기존에 잘 알려진 RCC의 종류인 clear cell RCC, papillary RCC, chromophobe RCC, CDC 등은 다루지 않는다.

본 론

최근에 기술 된 RCC 중 비교적 증례가 많아 새로운 RCC의 분류로 가능성이 있겠다고 간주된 종류가 여기에 해당된다.

1. 최근에 새롭게 기술된 RCC
1) Tubulocystic RCC

Tubulocystic RCC는 최근에 기술된 종양으로 주로 피질 또는 피질과 수질의 경계부에 중심을 둔 종양으로 경계가 좋고, 주로 종양 피막으로 잘 둘러싸고 있으며, 회백색의 단면소견을 보인다. 현미경 소견상, 한 층의 상피세포로 구성된 다양한 크기의 낭종성 또는 관상 구조로 구성되어 있다(Fig. 1). 이 낭종을 싸고 있는 종양세포는 호산성의 세포질을 가지고, 구두징 모양의 세포핵을 보이며 핵소체가 뚜렷하다[4]. 저등급 종양임에 반해 조직학적으로는 Fuhrman nuclear grade 3에 주로 해당하는 세포핵을 보이며, 유사분열은 별로 관찰되지 않는다[4]. 면역조직화학검사상 CK8, CK18, CK19 과 AMACR 에 양성이고, CK7에는 가끔 양성을 보인다[4-6]. Tubulocystic RCC의 기원세포는 집합관(collecting duct)이라고 알려졌기 때문에 tubulocystic RCC가 처음에는 매우 분화가 좋은 CDC라고 생각되었다. 하지만, 면역조직화학검사 소견, 전자현미경 소견, 유전자 발현 연구를 통해서 집합관이 아니라 근위 곡뇨세관이나 사이세포에서 기원한다는 것이 알려졌다[4,7]. Tubulocystic RCC는 면역조직화학염색상이나 유전자 발현 연구를 통해 papillary RCC와 매우 비슷하다는 사실이 알려졌다[6]. 게다가 한 연구에 의하면 12 케이스의 tubulocystic RCC 중 10 케이스가 papillary RCC와 연관이 되어 있음이 알려졌다[8]. Tubulocystic RCC라는 용어는 위에서 언급한 전형적인 육안소견과 현미경소견이 보일 때만 진단해야 되며 papillary RCC나 CDC에서 부분적으로 tubulocystic 형태가 보일 때는 진단하지 말아야 한다. 대부분의 증례가 예후가 좋다. 수술한 증례는 대게 low stage (pT1, pT2)이고, 국소재발이 발생하거나[6], 드물지만 림프절, 간, 뼈 등으로 전이가 보고 되기도 한다[4,6,8,9].

2) Thyroid-like follicular carcinoma of kidney

Thyroid-like follicular carcinoma of kidney는 2006년 Jung 등[10]에 의해 처음 기술된 이후 현재까지 약 10여 케이스만 보고되었다[10-14]. 보고에 의하면, 종양은 경계가 좋고, 육안소견에서 갑상샘 실질을 닮았으며, 현미경 소견상 갑상샘 소세포암종을 연상시킨다(Fig. 2) [11]. 면역조직화학검사에서 thyroid transcription factor-1이나 thyroglobulin에 음성을 보이고, 비교적 RCC에서 자주 염색되는 CK7, PAX2, PAX8에 대해서는 자주 양성 결과를 보인다. 주로 저등급이나, 림프절이나 폐로 전이가 보고되었다[11,12]. 연령대는 다양하게 분포하며 주로 30~40대에 발생하며 남녀 비율은 비슷하다[10-14]. 부분적으로 유두상 모양을 띠는 종양도 보고 되고 있는데[15,16], 이런 증례들은 원칙적으로 배제되어야 한다. 그리고, 갑상샘에서 생긴 암종이나 난소에 생긴 갑상선 암종이 신장으로 전이한 증례를 반드시 감별해야 한다. 말기 신부전증이 되면 신장 갑상샘화가 되어 비슷한 모양을 보일 수 있으나 이때에는 세포들이 나빠 보이지 않고 분명한 종괴를 형성하지 않기 때문에 쉽게 감별할 수 있다.

3) Acquired cystic kidney disease-associated RCC

말기 신부전증이 되면 신장의 관들이 변화가 일어나는데 이를 후천성 신낭포질환(acquired cystic kidney disease)이라고 한다. 빈도는 떨어지지만 RCC가 가끔 후천성 신낭포질환에서 생기는데 그 위험도가 일반인들에 비해 100배가 넘는다[17-20]. 후천성 신낭포질환에서 생기는 RCC의 종류에는 clear cell (conventional) RCC, papillary RCC, chromophobe RCC와 같은 후천성 신낭포질환이 없는 사람에게 발생하는 종류의 RCC도 발생하지만 후천성 신낭포질환에서만 특징적으로 발생하는 acquired cystic kidney disease-associated RCC와 clear cell papillary RCC와 같은 비교적 드문 종류의 RCC가 있다. Acquired cystic kidney disease-associated RCC는 반드시 말기 신부전증이나 후천성 신낭포질환에서만 생기는 반면, clear cell papillary RCC는 말기 신부전증이 없는 환자에서도 발행할 수 있다[21-23]. 투석을 받은 기간과 후천성 신낭포질환과 신장암의 발생과는 직접적인 관련이 있다고 알려져 있다[23]. Acquired cystic kidney disease-associated RCC는 주로 만성신부전 환자의 추적 검사 시 우연히 발견된다[23]. 낭(cyst)들은 보통 그 수가 많고 대부분 단방낭이다. 종양은 낭의 벽에서 결절로 생기며, 가끔은 낭을 전부 채우기도 하며, 낭과 떨어져 고형성 종양으로 생기기도 한다. 육안 소견은 진한 노란색을 띠며 출혈과 괴사가 동반된다. 20%이상에서 양측에서 발생하며 다발성으로 생기는 경우는 50%가 넘는다[23]. 현미경 소견상, 종양은 단순포상선(acinar), 폐포상(alveolar), 고형상(solid), 낭상(cystic), 유두상(papillary) 구조적 패턴을 보인다. 암세포는 풍부한 과립성, 호산성 세포질을 가지고 있으며 핵은 크고 핵소체는 뚜렷하다. 체모양(sieve-like) 구조가 특징적으로 관찰된다. 그리고 대부분의 예에서 이 종양의 특징적인 소견이라고 할 수 있는 수산 칼슘 결정(oxalate crystal)이 관찰된다(Fig. 3). 이 수산 칼슘 결정은 다른 종양에서는 잘 발견되지 않는다[23,24]. 면역조직화학검사에서, AMACR에 강양성을 보이나, CK7에는 음성이거나 부분적으로 약하게 염색이 된다. 또 CD10, RCC antigen, glutathione S-transferase A에 대해 염색이 된다는 보고가 있다[25]. 일반적으로 만성신부전 환자로 적극적인 추적검사를 하는 중에 비교적 조기에 발견되므로 예후는 좋은 편이나, 드물게 전이를 하는 예후가 좋지 않은 예가 보고 되어 있다. 특히 말기 신부전증 환자에서 생기는 여러 종류의 종양 중에서 acquired cystic kidney disease-associated RCC는 더 예후가 좋지 않으며, 육종변화(sarcomatoid transformation)를 보이는 경우 더욱 예후가 좋지 않다고 알려져 있다[26].

4) Clear cell papillary RCC

Clear cell papillary RCC는 비교적 최근에 기술된 종양으로 처음에는 말기 신부전 환자에서 처음 기술되었으나 그 후 산발적으로 정상 신장에서도 보고되고 있다[21,23,27]. 보고된 증례가 별로 많지 않아 속단하기는 어렵지만, 이 종양은 전이가 된 증례는 없는 것으로 보고되고 있어, 다른 RCC에 비해 예후가 매우 좋은 것으로 생각된다[21-23,27-31]. 그렇지만 최근에 들어 이 종류의 RCC가 그렇게 드물지 않다는 경험을 저자 중 한 명이 경험하고, 그 경험을 최근에 보고한 바 있다[32]. 육안적으로 경계가 좋고, 주로 피막으로 잘 둘러싸여 있다. 단면소견은 노르스름한 회백색을 띠며 고형성 또는 낭성 변화를 보인다. 종괴는 비교적 크기가 작고 단발성으로 생기며, 한 쪽 신장에만 주로 발생하나 후천성 신낭포질환이 있는 경우 양측에 생기거나 다발성으로 생기는 경우가 보고되고 있다[23,24,33]. 현미경소견상, 다양한 관상(tubular), 단순포상선, 유두상, 낭상 구조를 보이며 세포질은 비어있고 투명하며 핵은 저등급이다. 가장 특징적인 조직학적 소견은 기저막에서 떨어져서 핵이 일렬로 배열하는 것이다(Fig. 4). 이러한 특징은 다른 조직학적 성장양식을 보이는 것과 상관없이 이 종양을 인지하는데 매우 도움을 주는 소견이다[21,22]. 유두상 구조는 거의 항상 관찰되고, 일부에서는 유두상 구조가 빽빽하게 차 있어 고형성 종양처럼 보일 수 있다. 관상 구조물이 대부분인 경우도 있는데 이 때문에 예전에 tubulopapillary RCC로 명명되기도 했다. 그 외 다른 성장양식으로 낭상, 폐포상/둥지형(nested), 그물모양(retiform)의 모양을 보일 수 있다[21,27]. 섬유성 간질이 종양 결절을 나누고 있는 모습이 가끔 관찰되고, 일부에서는 피막에 과도한 유사근육화생(myoid metaplasia)이 생겨 종양 결절을 둘러싸는 모습이 관찰된다. 이러한 특징적인 소견으로 말미암아 Hematoxylin and Eosin (H&E) 슬라이드 하에서도 clear cell RCC와 감별이 가능하나 두 질환의 면역조직화학염색 소견이 다르므로, 일부 감별이 어려운 예인 경우에는 면역조직화학검사가 도움이 된다[21,23,27,28,31]. 거의 모든 clear cell papillary RCC 예에서 CK7이 발현되며, CA-IX가 세포막을 따라 염색이 되는데 관강 내 가장자리(luminal border)에는 염색이 되지 않는, 소위 물컵 모양의 염색을 보이는 것이 특징적이며, 조직학적으로 유사한 conventional (clear cell) RCC에서는 모든 면이 염색되어 상자 모양을 보여 감별에 도움을 준다[31]. AMACR, CD10는 대개 음성을 보이고, 34βE12에 대해서는 흔히 양성을 보인다. 현재까지의 이 종양의 정의에 의하면, 조직소견이 비슷하더라도 특징적인 면역조직화학 염색 소견(CK7+ and AMACR -, CD10-)이 없을 경우 clear cell papillary RCC라고 진단하지 않도록 권장하고 있다. 위에서 언급한 것처럼 clear cell papillary RCC에 평활근육 구성성분이 동반 될 때 다음에 언급할 renal angiomyoadenomatous tumor (RAT)으로 진단해야 하는데 좀 더 많은 증례들을 모아 조사하여 두 종양이 같은 종양의 일종인지 전혀 다른 것인지가 밝혀져야 한다고 생각한다.

2. 잠정적인 RCC

새로 보고된 RCC 중 보고된 예가 너무 적어 새로운 RCC으로 간주하기에는 아직 이르다는 ISUP 비뇨병리의사들의 의견들이 많아 잠정적으로 RCC의 형태로 분류한 종양들의 종류를 기술한다.

1) RCC with t(6;11) translocation

유전자 전위가 있는 RCC의 아집단 중에서 가장 잘 알려지지 않은 것이 t(6;11) (p21;q12)이 관찰되는 아집단이다. 이 부위의 전위로 인해 Alpha-TFEB 유전자의 융합이 일어난다. 이 특징적인 종양은 2001년 처음 보고 된 이후[34], 유전적으로 확진된 증례 보고는 30여 개 밖에 되지 않는다[35-41]. RCC with t(6;11) translocation는 주로 젊은 사람에게서 생기고 일반적으로 예후가 양호하다. 전이가 보고된 증례는 매우 드물다. 조직학적으로 가장 많이 발견되는 소견은 이중소견(biphasic morphology)으로 투명하거나 약간 호산성을 띠는 풍부한 세포질을 가지는 큰 상피모양세포와 주로 유리질 물질 주위로 뭉쳐 rosette 형태를 보이는 작은 세포로 구성된다(Fig. 5). 하지만 다른 모양을 가지기도 하여, papillary, tubular, chromophobe RCC, clear cell RCC, epithelioid angiomyolipoma-like 구조를 보이는 경우도 보고되어 있다[35,37,38,40-45]. RCC with t(6;11) translocation는 clear cell RCC와 다르게 HMB-45와 같은 멜라닌 세포성 표지자를 발현하는 반면, 상피 표지자인 cytokeratin에는 음성이거나 약하게 염색이 되는 특징을 보인다. 특히 거의 모든 증례에서 cysteine protease cathepsin-K에 염색이 되는데, RCC with t(6;11) translocation에 매우 특이적인 소견이다[35,39]. 이 cysteine protease cathepsin-K는 뼈파괴세포와 일부 Xp11 translocation RCC에서만 발현되고 다른 흔한 RCC에서는 발현되지 않는다[39]. Alpha-TFEB 유전자의 융합으로 인해 TFEB의 과발현이 생기고 이는 면역조직화학염색으로 핵에 염색이 되는 것으로 확인할 수 있다. 그러나 이 염색은 매우 조직 고정상태에 따라 유동적이어서 판독이 어려울 수 있는 단점이 있다. 요즘은 break-apart FISH 방법이 개발되었다[36]. 지금까지 보고된 약 30개의 증례 중에서 약 3 증례에서 전이가 관찰되었다[46]. 주로 젊은 어른에게서 발생한다. TFEB 면역조직화학검사와 break-apart FISH방법을 이용하여 더 많은 증례가 진단될 것으로 보인다. 이 종양을 진단하기 위해서는 30세 이전에 발생하는 RCC에서는 반드시 cytokeratin 염색을 한 뒤 음성이거나 약양성이면 염색체 전위로 발생한 RCC를 의심하여야 하고, TFE3 염색을 하여, 양성이면 Xp11 translocation RCC로 진단하고, 만일 음성이면 TFEB와 HMB45염색을 하여 t(6;11) translocation RCC로 진단하도록 추천된다[47].

2) Hybrid oncocytoma/chromophobe RCC

Hybrid oncocytoma/chromophobe RCC (HOCR)는 chromophobe RCC와 oncocytoma의 형태적 특징을 같이 가지고 있는 종양을 일컫는다. HOCR는 처음에는 renal oncocytosis/oncocytomatosis와 연관되거나, Birt-Hogg-Dubé (BHD) syndrome 환자에서 발생하는 것이 처음 알려지기 시작하였으나, 점차로 산발적으로도 발견되는 예가 보고되었다[48-50]. 연구에 따르면 이 세 그룹 모두 조직소견은 비슷하나, 분자유전학 배경은 다르다[48-56]. BHD syndrome 환자는 상염색체 우성 유전병으로 HOCR 신장종양 외에 섬유털집종(fibrofolliculomas), 폐낭종, 자발성 기흉 등이 동반되는 것이 특징적이다[54]. 이 환자의 종양은 oncocytoma와 chromophobe RCC가 함께 있는 형태학적 특징이 있다. 이 환자는 다른 RCC의 발생도 증가한다. 산발적으로 발생하는 HOCR는 주로 한 쪽에만 그리고 하나의 종양이 생기는 반면, BHD syndrome이 있는 환자나 oncocytosis/oncocytomatosis 환자에서는 많은 경우 양측, 그리고 다발성으로 발생한다[46]. 산발적으로 발생하거나 oncocytosis/oncocytomatosis 환자에서 발생하는 HOCR는 조직소견이 비슷하다. 종양세포는 고형성-폐포상(solid-alveolar) 형태를 보이고 핵은 약간의 다형성을 보이며, 풍부한 과립성, 호산성 세포질을 갖는다(Fig. 6). 자주 핵 주위의 달무리(halo)를 가지고 있다. 보통 renal oncytoma을 닮았다.

반면, BHD syndrome 환자에서 보이는 HOCR은 3가지의 특징적인 형태가 있다: (1) 전형적인 renal oncocytoma와 chromophobe RCC 가 섞여있거나, (2) 전형적인 renal oncocytoma 배경에 chromophobe 세포가 흩어져 있거나, (3) 세포내 공포를 가지는 큰 호산성 세포를 보이는 특징이 있다. 이 종양에서의 핵은 더 다형성을 보이고, 가끔씩 raisinoid 모양을 보인다[51,54,57,58]. 면역조직화학검사상 대부분의 종양이 parvalbumin, antimitochondrial antigen, CK7에 양성이며, CD117에 대해서는 다양한 결과를 보인다[46]. 산발적으로 발생하는 HOCR에 대한 전자현미경 연구에서 종양세포는 다양한 크기의 많은 미토콘드리아가 있다[49]. HOCR은 일반적으로 예후가 좋으며, 나쁘더라도 낮은 악성도를 갖는다[50]. 이번 ISUP 회의에서 대부분의 참가자들은 HOCR를 chromophobe RCC의 하위범주로 인지한다고 알려졌다.

3) Hereditary leiomyomatosis and RCC syndrome

Hereditary leiomyomatosis and RCC (HLRCC)는 상염색체 우성 유전을 하는 유전질환으로 피부와 자궁에 근종과 RCC를 특징으로 한다[59]. 이 증후군의 특징은 fumarate hydratase (FH, 1q42.3-q43) 유전자의 돌연변이이다. 이 증후군이 있는 환자에서 RCC는 약 1/3에서 발생하며, 산발적으로 발생하는 type2 papillary RCC 또는 CDC를 닮았다[46]. 다른 유전적으로 생기는 RCC증후군과 다른 점은 주로 RCC가 하나만 생기지만 매우 예후가 좋지 않다는 것이다. 대부분의 환자에서 많이 진행이 된 상태에서 발견되고, 이로 인해 사망하게 된다[60]. 형태학적으로 유두상 구조가 가장 흔하지만, 관상-유두상, 관상, 고형성, 혼합성 형태가 관찰되며, 특징적으로 세포는 매우 뚜렷한 핵소체가 있으며 그 주위로 달무리가 관찰되어 마치 거대세포바이러스의 핵처럼 보인다(Fig. 7) [61]. HLRCC는 케이스가 적지만 ISUP참가자들은 HLRCC를 하나의 특징적인 종양군으로 인지하였고, 특히 다른 유전적인 RCC와는 다르게 3 cm 이하의 작은 종양의 경우도 예후가 매우 나쁘기 때문에 추적검사를 철저히 하여야 하고 수술적인 치료를 고려해야 한다는 의견이었다. Papillary RCC인데 핵소체가 지나치게 뚜렷하고 크면 HLRCC를 생각하고 환자의 병력을 잘 관찰하여 그 전에 자궁이나 피부에 근종의 병력이 있나 조사하고 있으면 그 전 표본을 조사하여 근종에도 이상적으로 큰 핵소체가 보이는지를 확인할 필요가 있다. 또 다른 한편으로는 자궁 근종이나 피부 근종이 있는 환자의 근종 조직소견에서 바이러스 봉입체처럼 지나치게 큰 핵소체가 보이면 RCC의 가능성을 임상의사에게 제시하여 검사하도록 하는 것을 추천한다.

4) Renal angiomyoadenomatous tumor

Michal 등[62]이 보고한 5예의 renal angiomyoadenomatous tumor (RAT)의 기술을 보면, 종양은 투명세포, 평활근 기질, 샘종 구조를 가지며 샘종 구조는 소위 말하는 상어의 이빨을 닮은 모양이다(Fig. 8). 면역조직화학 염색상 EMA, CK7, CK20, AE1-AE3, CAM5.2, vimentin 등에 양성이다[62]. 하지만 clear cell papillary RCC의 경우에도 평활근화생으로 말미암아 angioleiomyoma양 증식을 보이는 경우가 보고되고 있고 이 경우 상당히 RAT와 유사한 모양을 보인다[63,64]. 그래서 아직까지는 이 두 종양이 같은 종양인지 다른 종양인지에 대해 논란이 있다[65-69]. 우리는 최근에 한 환자에서 두 가지 종양이 각각 다른 신장에 발생하였는데 두 종양이 같은 기원인지, 아니면 전혀 다른 종양인데 clear cell tubulo-papillary tumor components가 두 종양에서 비슷하게 보이는지를 알기 위해 comparative genomic analysis를 진행하고 있다.

결 론

최근 RCC에 대한 분자생물학적 과정에 대한 이해에 많은 발전이 있었다. 이러한 연구를 바탕으로 특정한 유전자 변화와 병리학적 특성간의 연관성 및 진단과 예후인자, 그리고 치료에 대하여 연구가 진행되고 있다. 이번 종설을 통해 최근에 분류되고 있는 새로운 RCC의 질병 군을 소개함으로써 진단하는 병리 의사뿐 아니라 임상의사에게도 환자의 예후 판정과 치료에 유용한 도움이 되는 가이드라인으로 사용되었으면 한다.
