갑상선암의 분자표지자들에 관한 고찰

Abstract

The incidence of thyroid cancer has been rapidly increased in Korea. Although fine needle aspiration cytology is recommended for diagnosis of cancer, there are some limitations. Patients with indeterminate or suspicious cytology category in which malignancy cannot be ruled out usually undergone a thyroidectomy, however, only 10∼25% of them finally diagnosed as cancer. According to the progress in understanding molecular mechanism, some mutations or other molecular alterations have been studied for the diagnostic and prognostic markers for thyroid cancer. The majority of papillary thyroid cancers have BRAF and RAS mutations or RET/PTC rearrangement, and approximately 80% of follicular thyroid cancers harbor a RAS mutation or PAX8/PPARg rearrangement. These genetic alterations are mostly studied and current clinical guidelines suggested that these molecular markers may help management for patients with indeterminate cytology. In addition, recent studies demonstrated the high sensitivity and specificity of thyroid-stimulating hormone receptor mRNA in diagnosing cancer in patients with indeterminate cytology. For the detection of recurrent or residual thyroid cancer, serum thyroglobulin is the only circulating marker in clinical practice. However, it lacks sensitivity and is unreliable specifically in the presence of antibodies to thyroglobulin. Recent studies demonstrated a significant role of measuring the mRNA of thyroglobulin, thyroid peroxidase, thyroid-stimulating hormone receptor, and sodium/iodine symporter in peripheral blood for monitoring of the recurrence of thyroid cancer.

서 론

갑상선암은 내분비계에 발생하는 가장 흔한 악성 종양으로 여성이 남성에 비해 3배 정도 많이 발생하는 것으로 알려져 있다. 최근 전 세계적으로 갑상선암의 발생률이 급증하는 것으로 보고되고 있으며, 우리나라의 경우 그 증가율이 매우 높아서 2008년 현재 여성암 발생률 1위를 보이고 있다[1]. 또한, 우리나라의 갑상선암은 다른 나라에 비해 유두암의 빈도가 높고 BRAFV600E 양성률이 높은 특징을 보인다[2]. 갑상선결절은 초음파유도 세침흡인세포검사를 통해 진단하는 것이 가장 정확하고 비용-효율적으로 알려져 있으며, 결절의 5% 정도가 악성으로 진단된다[3]. 분자생물학 기법의 발달로 다양한 생물학적 표지자가 갑상선암을 진단하고 예후를 예측하며, 수술 후 경과를 관찰 하는 데에 연구되고 있다. 특히, 대한갑상선학회의 2010년 ‘대한갑상선학회 갑상선결절 및 암 진료 권고안 개정안’과 미국갑상선학회의 2009년 ‘갑상선결절 및 암 진료 권고안의 개정안’에서는 갑상선결절의 진단 정확성을 높이기 위하여 v-raf murine sarcoma viral oncogene homolog B1 (BRAF), rat sarcoma viral oncogene homolog (RAS), rearranged during transfection/ papillary thyroid cancer (RET/PTC), paired box gene 8/peroxisome proliferator-activated receptor (PAX8/ PPARg), galectin-3, cytokeratin 등의 분자표지자 검사를 전문가 의견에 따른 권고사항으로 분류하고 있다[4,5]. 이번 종설에서는 갑상선암에서 발견되는 유전자변이와 이를 이용한 진단과 추적관찰에 대해 다루고자 한다.

본 론

1. 갑상선암에서의 주요 유전자변화

갑상선암은 조직학적으로 갑상선 분화암, 갑상선 미분화암 및 갑상선 수질암으로 분류되며, 갑상선 분화암은 다시 유두암과 여포암으로 분류된다. 갑상선 분화암과 관련된 4개의 주된 돌연변이로 BRAF, RAS, RET/PTC, PAX8/PPARg가 알려져 있다.

1) 유두암

유두암의 약 75%에서 BRAF, RAS, 또는 RET/PTC 돌연변이가 관찰되며 mitogen-activated protein kinase (MAPK) 경로를 활성화 시킨다[6]. 특히, BRAF 돌연변이와 RET/PTC 돌연변이는 상호 보완적으로 발견된다[7].

(1) BRAF: BRAF 돌연변이는 유두암에서 가장 흔하며 BRAF kinase와 MAPK/extracellular signal-regulated kinase (ERK) 경로를 지속적으로 활성화시킨다[8]. 가장 흔하게 발견되는 변이는 600번 아미노산인 발린이 글루탐산으로 치환된 것(V600E)으로 1799번 뉴클레오티드의 점돌연변이가 원인이다[9]. BRAFV600E 돌연변이는 키가 커지고 여포성 변이를 보이는 세포거나 갑상선 미분화암에서도 발견되지만, 여포선종이나 여포암에서는 발견되지 않는다[10].

(2) RET/PTC: RET/PTC 재배열은 RET 유전자의 tyrosine kinase영역인 3’끝과 PTC 유전자의 5’끝이 융합된 결과이다[11]. 이 융합유전자에서 만들어진 RET/ PTC chimeric 단백은 세포 내 tyrosine kinase 부위가 지속적으로 활성화되어 있다. 그 결과 다양한 하향 경로인 MAPK, ERK, phosphatidyl inositol 3-kinase (PI3K) 경로를 활성화시킨다[12]. 갑상선암과 관련된 RET/PTC 유전자 재배열은 RET/PTC1RET/PTC3이 가장 흔한 것으로 알려져 있으며, 산발적으로 발생하는 유두암의 15∼40%에서 발견된다[13]. 이 유전자 재배열은 소아의 유두암과 방사선 노출에 의한 유두암에 관련된 것으로 생각되고 있다[14,15]. 이는 유두암의 여포성 변이에서도 발견되며 일부 연구자들은 여포선종에서도 보고하고 있다[16,17]. RET/PTC1은 tyrosine kinase receptor인 RET 유전자와 기능이 알려지지 않은 H4 유전자의 재배열이며, RET/PTC3은 역시 기능이 알려지지 않은 elephant ear-like leaf 1 (ELE1) 유전자와의 재배열이다. RET/PTC 유전자변화가 있는 유두암은 전형적인 조직형을 보이며 림프절 전이율이 높으나 미분화암으로 진행하는 경우는 적은 것으로 알려져 있다[6,18].

2) 여포암

여포암의 약 80%에서 RAS 돌연변이나 PAX8/ PPARg 재배열이 관찰된다.

(1) RAS: RAS 유전자에 의해 만들어진 g-단백은 세포막수용체 들과 연관되어 이들이 활성화되면 MAPK, PI3K/v-akt murine thymoma viral oncogene (AKT) 경로의 신호전달과 활성화를 촉진하는 것으로 알려져 있다[19]. RAS 유전자에 점돌연변이가 일어나면 이들 경로가 지속적으로 활성화된다. 갑상선암의 발생과정에는 HRAS의 61코돈과 NRAS의 61코돈의 돌연변이가 가장 흔히 발견되는 것으로 알려져 있다[20,21]. RAS 돌연변이는 유두암과 여포암 모두에서 발견되며 여포선종에서도 상당수 발견되는 것으로 보고되고 있다[20,22]. RAS 변이를 갖고 있는 유두암은 여포성 변이 부르는데, 림프절 전이가 적고 피막에 둘러싸인 경향을 보인다는 보고가 있다[23].

(2) PAX8/PPARg: PAX8 유전자는 갑상선에서 특이 발현되는 전사인자를 암호화하고 있으며, PPARg 유전자는 peroxisome proliferator activated receptor g를 발현한다[24]. PAX8/PPARg 재배열은 이들 두 유전자의 전이와 융합으로 만들어지며 세포 변형에 관여한다[25]. PAX8/PPARg 재배열은 전형적인 여포암의 30∼40%에서 보이며 여포선종의 2∼10%에서 발견된다[26-29]. PAX8/PPARg 재배열이 있는 종양은 고형, 둥우리의 성장 패턴을 보이며 종종 혈관 침범을 동반한다[30]. PAX8/PPARg 재배열은 유두암의 여포성변종에서도 발견된다[26].

(3) Micro-RNA: Micro-RNAs (miRNAs)는 non-coding RNAs로 유전자 발현을 음성적으로 조절한다. miR-155, miR-181b, miR-213, miR-221, miR-222, miR-224, miR-345는 유두암에서 조절이상이 보고되고 있다[31-34]. 또한, 종양의 돌연변이 상태와 miRNA 개요간의 연관성도 제시되고 있다[35]. miRNA 조절이상은 여포암[35]이나 역분화 갑상선암[34]에서도 보고되고 있다.

2. 진단
1) 갑상선기능검사와 영상진단

갑상선결절이 발견되면 갑상선 기능검사를 시행한다. 그 결과, thyroid-stimulating hormone (TSH)이 높으면 악성의 가능성이 거의 없으므로 세침흡인세포검사를 시행하지 않을 수 있고 TSH가 낮으면 갑상선스캔을 시행하여 열결절인지를 확인한다. 혈청 TSH가 낮지 않다면 갑상선스캔을 생략하고 갑상선초음파를 시행하여 결절의 크기, 위치, 악성이나 양성을 시사하는 소견, 낭성 변화 등을 확인할 수 있다. 혈청 TSH가 증가되어 있는 하시모토 갑상선염 환자들은 갑상선암의 빈도가 정상 갑상선과 비교하여 같거나 높기 때문에 세침흡인세포검사를 권고하고 있다[5]. 또한, 갑상선결절이 존재하거나 의심되는 모든 환자에서 갑상선초음파 시행을 고려하도록 권고하고 있다[5].

2) 세침흡인세포검사

2009년에 발표된 Bethesda 체계는 세침흡인세포검사 결과를 비진단적, 양성, 비정형, 여포종양 혹은 여포종양 의심, 악성 의심, 악성으로 분류하고 있다[36]. 이들의 악성 위험도는 각각 양성은 1% 미만, 비정형은 5∼15%, 여포종양은 15∼35%, 악성 의심은 50∼80%, 악성은 97∼100%로 알려져 있다[36-43]. 세침흡인세포검사의 약 7∼10%의 경우는 의심이나 불명확으로[44,45] 이들 환자들은 진단이 명확하지 않고 악성 위험도를 예상할 수 없어 대부분의 경우 수술을 하게 된다. 그러나 수술한 환자의 10∼25%만이 악성으로 최종 진단된다[44,46,47]. 따라서, 많은 수의 환자들이 불필요한 수술을 받으며, 전절제술을 받은 환자의 경우 영구적인 저갑상선증으로 매일 갑상선호르몬을 복용해야 한다. 또는 보존적인 수술을 우선 시행하여 진단 목적의 갑상선엽절제술만을 시행받고 악성으로 진단된 환자들은 이차 수술을 통해 갑상선을 완전 절제하여야 한다.

3) 유전자 변화의 분자생물학적 검사

세침흡인세포검사를 보완할 수 있는 검사로 유전자 변이에 대한 분자검사법이 향후 기대된다. 최근 발표된 전향적 연구들에서 BRAFV600E와 관련된 결과가 발표되었는데, 유두암에 대한 특이도가 100%였다[48-53]. 또한, 의심, 불명확, 또는 비진단적인 세침흡인세포검사 결과가 나왔을 때 BRAFV600E가 있으면 악성으로 분류하는데 도움이 되며, 결과가 양성으로 나왔더라도 BRAFV600E가 있다면 수술 후 악성으로 진단되는 경우가 드물지 않다고 보고하고 있다[54-59]. 특히, BRAFV600E를 혈액 내 순환하는 세포에서 진단하거나[54,60], 유두암으로 수술한 환자의 수술 전 혈액에서 진단한 보고들이 있다[61].

최근의 두 연구는 세침흡인세포검사 검체를 이용하여 BRAF, RAS, RET/PTC, PAX8-PPARg 유전자 변화를 조사하였다[52,53]. 그 결과 어느 하나의 변화라도 보이면 악성으로 확인되는 경우가 각각 97%와 91.1%였고, BRAFRET/PTC에 변화가 있는 경우 모두 악성으로 확인되었다. 유전자변이 검사가 양성인 경우 100%의 특이도로 악성임을 확인할 수 있었으나, 음성인 경우에도 악성을 배제할 수는 없었다고 하였다. 두 연구에서 각각 32%와 10%의 경우 유전자변이가 음성임에도 악성으로 확인되었던 환자들이 있었던 것으로 보고하였다. 이와 같은 연구 결과들로 볼 때 유전자변이 분석을 통해 갑상선 절제술의 시행을 줄이는 것은 아직은 어려울 것으로 보인다. 그럼에도 불구하고 유전자변이 분석을 통해 몇 가지 임상적 도움이 될 수 있는 경우들은 다음과 같다. 첫째, 갑상선엽절제술과 전절제술의 결정; 둘째, 수술 중 동결절편검사의 필요성이 감소; 셋째, 세침흡인세포검사 결과가 의심 또는 불명확인 환자들이 유전자 변이 양성일 경우 수술의 결정; 넷째, 세침흡인세포검사 결과가 의의가 불확실한 여포성 병변이거나 검체가 불충분할 때 향후 치료 결정; 다섯째, 수술 전에 절제술의 범위를 결정하는 데에 도움이 될 수 있다[62].

유전자 변이 외에도 Wagner 등[55]은 처음으로 결절의 진단시 TSH receptor (TSHr) mRNA를 연구한 결과를 발표하였다. 72명의 환자에서 수술 전에 TSHr mRNA를 정량 RT-PCR로 측정하였는데, 그 결과 갑상선분화암 환자의 72%를 정확히 진단하였고 양성결절 환자의 80%를 정확히 진단하였다. 세침흡인세포검사가 불명확한 환자의 진단에서 4명의 암환자 중에 3명의 암환자를 정확히 분류하였고, 14명 양성결절 환자 중에 11명을 정확히 분류하여 민감도 75%와 특이도 78%를 보여주었다. 또한 세침흡인세포검사와 TSHr mRNA RT-PCR을 같이 하였을 때는 진단의 민감도가 95%이며 특이도가 89%로 세침흡인세포검사만 할 때 보다 높다고 주장하였다. Chia 등[56]은 이들의 연구를 보다 발전시킨 결과를 발표하였는데, 207명의 환자를 대상으로 TSHr mRNA를 정량RT-PCR로 측정하였다. 연구자들은 receiver operating characteristic (ROC) 커브를 이용한 기준치를 적용하여 갑상선분화암을 진단하는데 민감도 72%, 특이도 82.5%의 결과를 보고하였다. 세침흡인세포검사 결과 불명확의 결과를 보인 환자들 60명에서 TSHr mRNA검사는 68%인 41명의 환자들을 정확히 분류할 수 있었다. 따라서 TSHr mRNA검사가 수술 전 갑상선결절 검사에서 세침흡인세포검사를 대신하긴 어려우나 불명확한 세침흡인세포검사 결과를 보일 때 도움이 될 것으로 보고하였다.

요약하면, 현재는 세침흡인세포검사에서 비정형이나 여포종양 혹은 여포종양 의심의 경우 진단의 정확성을 높이기 위해 BRAF, RAS, RET/PTC, PAX8-PPARg 등의 유전자 변화에 대한 검사를 고려할 수 있다고 권고하고 있다[5]. 지금까지 갑상선분화암의 유전자변이 중에 70∼80%가 밝혀졌으며, 향후 새로운 유전자변이와 mRNA에 대한 결과들이 발표될 것으로 생각된다. 그러나 아직 표지자들의 결과가 연구자들간에 큰 차이가 있어 임상적인 유용성은 향후 연구가 계속되어야 할 것으로 보인다.

3. 예후와 추적관찰

수술 후 검체에서 유전자 변이를 검사하는 것은 환자의 예후 분석과 위험도를 분류하는 데에 도움이 될 수 있다. 이를 바탕으로 수술 후 약물 치료와 진행되거나 전이된 갑상선암 환자의 표적치료에 응용할 수 있을 것이다.

1) 말초혈액의 갑상선글로불린 측정

갑상선글로불린(thyroglobulin, Tg)은 갑상선 상피세포에서 합성하는 당단백이다. 대부분의 갑상선 분화암은 이를 합성하는 능력이 있어 여포상피세포 기원의 갑상선암 환자를 추적 관찰하는 가장 좋은 표지자로 이용되고 있다. 갑상선글로불린은 면역검사법으로 측정할 수 있다. 검사간의 차이를 줄이기 위해서 Tg 검사는 같은 제조사의 시약을 사용하여 같은 검사실에서 검사하는 것이 좋다. 또한 이들 검사의 결과는 anti-thyroglobulin antibody (TgAb)에 의해 영향을 받으므로 TgAb를 동시에 측정하는 것이 권장된다. 드물게 Tg 측정결과가 heterophil human anti-murine antibodies (HAMA)에 영향을 받기도 한다.

2) 분자 진단방법에 의한 수술 후 추적관찰

(1) BRAF, RAS 돌연변이 검사: 갑상선분화암은 대부분 임상 경과가 양호하지만, 빠르게 진행하는 경우도 있다. 아직까지는 진행이 느린 종양과 공격적인 종양을 구별하는데 어려움이 있다. 많은 연구들에서 BRAF 돌연변이가 종양의 공격성과 관계가 있다고 보고하였다. 또한 재발과 사망을 예측하는 독립인자라는 보고도 있다. 이와 관련된 설명으로 BRAF 돌연변이는 MAPK 경로를 활성화시키는데 이는 종양의 역분화를 일으킬 수 있는 것으로 보고되고 있다. 또한, BRAF 돌연변이는 sodium-iodine symporter (NIS) expression과 iodine 대사에 영향을 주기 때문에 종양이 방사선요오드치료에 저항성을 획득하는데 관련된 것으로 보고되고 있다. 그러나 BRAF 돌연변이가 공격적인 진행을 보일 것으로 기대되는 것보다 더 많은 수의 유두암 환자에서 발견되는 것은 BRAF 돌연변이 양성인 환자들에게 영향을 주는 다른 인자가 있음을 시사해준다. 따라서 BRAF 돌연변이만으로 위험분류를 하여 적극적으로 치료할 경우 많은 수의 유두암 환자들을 과잉 치료하게 될 위험이 있다. RAS 돌연변이는 여포암과 유두암에서 공격적인 진행과 관련된다는 보고들이 있으나, 양성종양과 진행이 느린 종양에서도 RAS 돌연변이의 발견이 비슷하여 이를 임상적으로 예후에 이용하는 것은 어려울 것으로 보인다.

(2) 말초혈액을 이용한 Tg, TPO, TSHr, NIS mRNA RT-PCR: 말초혈액 내에 특정 mRNA를 RT-PCR로 측정하는 방법들이 연구되고 있다. RT-PCR은 민감한 검사이므로 우리가 검사하기 원하는 mRNA의 특이도가 매우 중요한데, Tg, thyroid peroxidase (TPO), TSHr, NIS에 대한 mRNA측정이 연구되고 있다. 이 중에서 Tg mRNA에 대한 연구가 가장 많으며, 재발암 진단에 대한 특이도가 58∼100%로 연구마다 차이를 보이고 있다. 갑상선암의 재발의 예측에 mRNA의 혈액 내 측정이 더 민감하다는 연구결과[18,55,57,58]와 면역검사법이 더 민감하다는 연구결과[63]가 모두 보고되고 있다. Tg mRNA의 측정에 대한 문제점들은 건강한 사람에서도 측정되며, 국소 및 전이암 환자에 대한 특이도가 25∼30%밖에 되지 않으며[59,64-67], Tg mRNA는 림프구 등에서도 전사가 되며[68], 갑상선세포에서 Tg mRNA가 전사되면서 상호짜집기[69]에 대한 문제가 있다. 아직까지 Tg mRNA에 대한 특이도 문제로 임상에서 활용하는 것은 어려운 상태이다. Chinnappa 등[70]은 적절한 시발체 선택으로 100%의 특이도를 얻었다고 보고하였다. Barzon 등[71]은 재발성 및 전이성 암을 발견하는데 Tg, NIS, TSHr, TPO mRNA연구 결과를 발표하였다. 그들의 연구에서 Tg의 민감도는 93%로 높았고, NIS mRNA의 민감도는 72.4%였다. 그러나 둘 다 특이도가 29.4%로 낮았다. TPO mRNA와 TSHr mRNA는 특이도가 64와 80.6%로 높았으나 민감도가 53과 40%로 낮았다. Ishikawa 등[68]은 33명의 분화갑상선암환자에서 TPO, Tg, TSHr mRNA를 연구하였다. TPO mRNA가 가장 특이도가 높았지만(96%) 민감도는 61%로 Tg에 비해 떨어졌다. Chinnappa 등[70]은 재발성 및 잔존암의 발견을 위해 말초혈액에서 TSHr mRNA를 측정하여 100%의 민감도와 98%의 높은 특이도를 보고하였다.

결 론

최근 갑상샘암의 발생과 진행에 관련된 분자생물학적 과정에 대한 이해에 많은 진전이 있었다. 이를 바탕으로, 특정한 유전자 변화와 조직학적 특징간의 연관을 분석하고 진단, 예후인자, 그리고 치료와 관련된 도구로 개발하기 위한 연구들이 진행되고 있다. 아직까지 진단에 있어 유전자 변화에 대한 검사는 세침흡인세포검사에 보조적인 역할로 한정되고 있으나, 여러 유전자 변화들을 분석하여 민감도와 특이도를 높이기 위한 연구들이 이루어지고 있다. 수술 후 재발과 전이에 대한 추적관찰에는 현재 혈청 Tg를 측정하고 있으나 검사방법에 따른 특성과 Tg 항체에 따라 결과의 신뢰성에 문제가 있다고 알려져 있다. 이를 극복하기 위해서 혈액 내 검출되는 갑상선에 특이적인 mRNA가 연구되고 있으며, TSHr mRNA와 TPO mRNA 등의 유용성이 발표되었다. 특히, 말초혈액에서 TSHr mRNA의 측정은 재발암과 잔존암의 발견에 높은 민감도와 특이도를 보고하고 있어 향후 추가적인 연구결과들이 기대된다.
